Why dry eyes?blepharitis causes dry eye syndrome

Dry Eye incidences are increasing due to stress, constant electronic devices and aging.

We have now observed that inflammation of the eyelids are occuring in a very high proportion of people with dry eyes.

Inflammation of the eyelid margin is a common characteristic. This condition can be acute or chronic and is mostly encountered in adults.

When the lashes are affected, anterior blepharitis is most commonly the diagnosis.  When the meibomian glands that produce the vital oils and fluids that keep the eyes moist become affected, posterior blepharitis is found.

Causes and Symptoms

There are three causes:  staphylococcal infection, seborrheic dermatitis, meibomian gland dysfunction.  A combination of these can cause dry and irritated eyes.

In fact, most patients often have a combination of these causes, but one type may be predominate.

With five out of every one hundred patients have it, primary care facilities are taking notice.  However, many experts believe that the true number is even larger.

Staphylococcal is more common in women, but all forms can be found in both sexes.

It most commonly starts in a person’s 40s and 50s.

The symptoms include:

  • Itchy or burning eyes
  • Gritty or sore eyes
  • Morning symptoms that are worse for patients
  • Symptoms occurring in both eyes
  • Long periods of symptoms followed by long periods of remission
  • Watery eyes
  • Blurred vision
  • Dry eyes
  • Inability to wear contact lenses
  • Dandruff, oily skin, or facial rashes
  • Facial flushing or redness

Signs may include:  eyelid margins that are reddened, crusty, or scaling.  Eyelash deformity, depigmentation, or loss can be a sign of anterior blepharitis.  A greasy appearance on or around the eye lashes or lashes that are stuck together may be a sign of seborrheic blepharitis.  Dilated or obstructed glands tend to show posterior and meibomian gland dysfunction.

Clinical Diagnosis

A doctor may look at the following:

  • Slightly inflamed lid skin
  • Loss of lashes
  • Inflammation around the meibomian gland orifices
  • Deficient or foamy tear film
  • Conjunctivitis or chalazion formation
  • Corneal erosion, scarring, and neovascularization

Management

If it is determined, antibiotics alone rarely solve the problem.  Rather, the patient action is often more productive.  Avoidance of contact lenses, restricting use of eye make-up, and daily lid hygiene are actions a patient can take that will eliminate or relieve the symptoms.

Lid hygiene is especially important.  A warm compress placed on the eyes for 10 minutes- twice a day can help to loosen collarettes and diminish crusting on the meibomian glands.

Lid massage, most useful for posterior, loosens the meibomian gland content.  Lid cleansing sprayed onto a gauze pad is especially helpful in getting rid of the eye of debris and reduces margin inflammation.
