Age-related macular degeneration (AMD) affects the macula, the part of the eye that allows you to see fine details. This disease occurs mostly with aging, but can also happen to younger people through genetic inheritance. AMD is the leading cause of blindness with people over the age of 60.
In some people, vision loss in AMD can occur slowly. With others, vision loss can be rapid and damaging.
There are 2 types of AMD: Dry and Wet:
1. Dry AMD
This is the most common form of AMD, between 80-90% of all the AMD cases. Dry AMD often can lead to Wet AMD – where blood vessel burst leaking blood and fluid into the eye. Wet AMD is considered an emergency and often require immediate treatment at a hospital ( see section on Wet AMD below).
In the case of Dry AMD - the light-sensitive cells in the macula slowly break down, gradually blurring central vision in the affected eye. As dry AMD gets worse, you may see a blurred spot in the center of your vision. Over time, central vision can be completely lost.
The most common symptom of dry AMD is slightly blurred vision. You may have difficulty recognizing faces. You may need more light for reading and other tasks. Dry AMD generally affects both eyes, but vision can be lost in one eye while the other eye seems unaffected. One of the early signs of AMD is the presence of drusen bodies upon eye examination or eye photograph.
Dry AMD can develop into Wet AMD overtime. Wet AMD becomes a medical emergency and requires treatment immediately.
What are Drusen Bodies:
Scientists believe that drusen bodies are lipid and protein deposits under the retina. The increase of the number of drusen bodies can be an indication of worsening of the disease progression – developing into wet AMD or total vision loss.
New scientific research has also shown that the permeability of the Bruckner membrane, which regulates fluid and protein transport in the eye, is often affected. It is no longer functioning optimally to transport unwanted protein and fluids out of the eye to keep a healthy balance). The unwanted fluids and proteins accumulate becoming one of the primary cause of eye diseases in the aging eye- glaucoma, AMD, Diabetic Retinopathy.
Amsler Grid:
One of the signs of Dry and Wet AMD is seeing waving lines or blocked vision areas of an Amsler Grid. Amsler Grid is a square with many lines on it and a central foci. This is a part of your comprehensive eye examination where the doctor maps out where your wavy lines and blocked areas are – to track the progression of the disease.
2. Wet AMD
Wet AMD occurs when abnormal blood vessels behind the retina start to grow under the macula (angiogenesis). These new blood vessels tend to be very fragile and often leak blood and fluid. The blood and fluid raise the macula from its normal position at the back of the eye. Damage to the macula occurs rapidly when bleeding starts. The person looses vision almost instantly. You need to call your physician right away and get treatment.
An early symptom of wet AMD is that straight lines appear wavy. If you notice this condition or other changes to your vision, contact your eye care professional at once. You need a comprehensive dilated eye exam.
Treatments for Wet AMD
There are 3 types of treatments for Wet AMD- none cure the disease, and often needs to repeated to keep the bleeding from recurring:
- Laser surgery to seal off bleeding blood vessels behind the eye.
The laser may destroy some of the healthy tissues surrounding the blood vessels and leave scare tissues. The risk of new blood vessels developing after laser treatment is high. Repeated treatments may be necessary. In some cases, vision loss may progress despite repeated treatments.
- Photodynamic Therapy.
A drug called verteporfin is injected into your arm. It travels throughout the body, including the new blood vessels in your eye. The drug tends to stick to the surface of new blood vessels. A light is shined into your eye to activate the drug, which destroys the new blood vessels and leads to a slower rate of vision decline. Because the drug is activated by light, you must avoid exposing your skin or eyes to direct sunlight or bright indoor light for five days after treatment.
- Intra-vitreous Injections.
Wet AMD can now be treated with new drugs that are injected directly into the eye (anti-angiogensis therapy). Abnormally high levels of a specific growth factor occur in eyes with wet AMD and promote the growth of abnormal new blood vessels. This drug treatment blocks the effects of the growth factor.
You will need multiple injections that may be given as often as monthly. The eye is numbed before each injection. After the injection, you will remain in the doctor's office for a while and your eye will be monitored. This drug treatment can help slow down vision loss from AMD and in some cases improve sight.
Who is at risk for AMD.
Who is at Risk?
The greatest risk factor is age!
Although AMD may occur during middle age, studies show that people over age 60 are clearly at greater risk than other age groups. The risk increases dramatically for people over the age of 75.
Other risk factors include:
- Smoking. Smoking may increase the risk of AMD.
- Obesity.
- High cholesterol levels in the blood.
- Race. Whites are much more likely to lose vision from AMD than African Americans.
- Family history. Those with immediate family members who have AMD are at a higher risk of developing the disease.
- Gender. Women appear to be at greater risk than men.
